Parent’s Guide: Cough in Children

The Big Picture: A cough is not the enemy. It is a reflex that protects the lungs by clearing out germs and mucus. Most coughs are caused by viruses (like the common cold), meaning antibiotics will not help.

🚨 When to Get Help

Before treating the cough, strip your child down to the waist and watch them breathe.

Call 911 Now

  • Severe Struggle: Child is struggling for every breath or can barely speak/cry.
  • Color Change: Lips or face turn blue when not coughing.
  • Consciousness: Child has passed out or stopped breathing.

Call Doctor Now or Go to ER

  • Noisy Breathing:
  • Retractions: The skin pulls in between the ribs or at the neck with each breath.
  • Chest Pain: Severe chest pain, or unable to take a deep breath.
  • Blood: Child coughs up blood.
  • High Risk: Child has a weak immune system (Sickle Cell, HIV, Chemo) or chronic lung disease.
  • Fever: High fever (>104°F / 40°C).

Contact in 24hrs

  • Duration: Cough has lasted more than 3-4 weeks.
  • Ear/Sinus: Child complains of earache or sinus pain (cheekbone pressure).
  • Fever: Fever lasts more than 3 days.
  • Vaping: You suspect your teen is vaping and they have a cough.

🏠 Home Care Action Plan

Since most cough medicines are not recommended for young children, use these home remedies to soothe the airway.

🍯 The Best Medicine: Honey (Age 1+)

Honey has been proven to work better than drugstore cough syrups for thinning mucus and loosening a cough.

  • Dosage: Give ½ to 1 teaspoon (2 to 5 mL) of honey as needed.
  • Warning:Never give honey to a child under 1 year old (risk of Botulism).

💧 Hydration & Humidity

  • Fluids: Keep your child well-hydrated. This thins the phlegm in the lungs, making it easier to cough up. Warm apple juice or herbal tea can be very soothing.
  • Warm Mist: If your child has a “coughing fit,” take them into a bathroom with a hot shower running (door closed). Breathe the misty air for 10-15 minutes.
  • Humidifier: Dry air makes coughs worse. Run a humidifier in the bedroom.

🍬 Cough Drops (Age 6+)

  • For school-age children, sucking on a cough drop can stop the “tickle” in the throat.
  • Warning: Do not give to children under 6 years (choking hazard).

💊 What About Cough Syrup? (DM)

  • Under 6 Years: Do NOT use. They are not FDA approved for this age group and are not safe.
  • Over 6 Years: You can try a medicine with Dextromethorphan (DM), like Robitussin. However, honey often works better. Only use if the cough is severe and preventing sleep.

Understanding the Symptoms

“The Phlegm is Green!”

Parents often panic when clear mucus turns yellow or green.

  • The Truth: Yellow/Green phlegm is a normal part of healing from a viral bronchitis (cold). It means the immune system is working.
  • Antibiotics: Antibiotics do not fix green phlegm caused by a virus.

“My Child Vomited After Coughing”

  • This is common. A heavy cough triggers the gag reflex, especially if the stomach is full.
  • The Fix: Feed smaller amounts more often.

Allergic Coughs

  • If the cough is caused by cats, pollen, or dust (usually accompanied by itchy eyes/nose), an allergy medicine like Benadryl or Zyrtec can help stop it within an hour.

⏳ What to Expect

  • Viral Coughs: Typically last 2 to 4 weeks.
  • School: Your child can return to school once their fever is gone and they feel well enough to participate, even if they still have a mild cough.

⚠️ A Note on Vaping (Teens)

If your teen has a chronic cough, you must consider vaping.

  • Vaping can cause severe, permanent lung injury.
  • The chemicals in homemade or street-purchased vape juices are particularly dangerous.
  • Encourage your teen to stop immediately if they have a cough.
