How to Transfer Your Child’s Primary Care
- Notify Your Insurance: Call the member services number on the back of your insurance card to list an OC Children’s Medical Group provider as your child’s Primary Care Provider (PCP).
- Transfer Medical Records: Fill out our simple Medical Records Release Form. Send it to your previous pediatrician so they can forward your child’s history to
admin@occhildrens.combefore your first visit. - Schedule Your First Visit: Once you’re registered, book a well-child check or an initial consultation so we can establish a baseline for your child’s health.
